COVID-19 is a disease that has made a lot of people sick. COVID-19 has changed the way we live. Now, there is a vaccine for COVID-19. A vaccine is a shot that can help keep you from getting sick.
Getting a vaccine can be a big decision. We want to understand more about how people make decisions about the COVID-19 vaccine. We want to hear from people who decided to get the vaccine and people who decided not to. We also want to hear from parents of children who are too young to get the vaccine right now.
Getting a COVID-19 vaccine was hard for some people. We want to know more about what made it hard or easy to get the vaccine.
We hope to learn about how to help people with IDD stay healthy.
We will ask you some questions about what you think about the COVID-19 vaccine. We will also ask about your (or your family member’s) disability and health. We will also ask for basic demographic information about you (or your family member). Demographic information means things like age, race, and the state where you live. The survey will take about 15 minutes. You can ask someone to help you if you need it. You can also email the research team and set up a time to talk about the survey.
You need to be at least 18 years old to answer the questions. You need to be your own guardian to answer the questions. People with intellectual and/or developmental disabilities or their family members are welcome to participate.
Your answers are private. No one will know how you answer the questions. Your name will not be on your answers. You only need to give us your name and email if you want to be entered into a drawing for the gift card or if you want us to share what we learned. You don’t have to tell us your name or email if you don’t want to. We will not share your name or email with anyone. You should take the survey in a place that is private.
Participation in this survey is voluntary. Voluntary means you don’t have to answer the survey if you don’t want to. You can skip a question if you don’t want to answer it. If you change your mind and don’t want to answer any more questions you can stop.
